ASC Survey is an ASC compliance platform built with privacy and HIPAA compliance at its core. This policy describes how we collect, use, and protect your information.

Information We Collect

• Account information: Name, email, phone number, and facility details provided during registration.
• Usage data: Survey completion activity, compliance scores, and platform interactions.
• Demo requests: Name, email, phone, and role submitted through our booking forms.

We do not collect or store Protected Health Information (PHI) in logs. All PHI is handled in accordance with HIPAA requirements.

How We Use Your Information

• To provide and improve the ASC Survey compliance platform
• To respond to demo requests and support inquiries
• To send product updates and compliance-related communications
• To maintain audit trails as required by HIPAA

Data Security

All data is encrypted at rest and in transit. We enforce row-level security, maintain full audit trails, and implement zero-PHI logging policies. Our infrastructure is designed to meet HIPAA technical safeguard requirements.
